Terms and Conditions

• The New Adult Patient Prize Draw is open to new patients aged 18 years or over only.
• To qualify, eligible patients must book and attend their New Patient Examination and consultation between 1st July and 31st July.
• Children (patients under the age of 18) are excluded from the New Adult Patient Prize Draw.
• The Existing Patient Referral Prize Draw is open to registered patients of the practice who refer a new adult patient (18 years or over).
• Existing Patient Referral Prize Draw: Existing patients will receive one entry into the prize draw for every adult (18 years or over) friend or family member they refer who books and attends their New Patient Examination and consultation between 1st July and 31st July.
• There is no limit to the number of referral entries an existing patient may receive.
• The closing date for both prize draws is 11:59pm on 31st July.
• Winners will be selected at random on 3rd August and contacted using the details held by the practice.
• Prizes are non-transferable and no cash alternative is available.
• Smile Credit may only be used towards treatment provided at the practice and has no cash value.
• All dental treatment is subject to a clinical examination, diagnosis and suitability – acceptance of treatment cannot be guaranteed.
• Tooth whitening is subject to a clinical examination and suitability and cannot be exchanged for cash.
• The practice reserves the right to substitute prizes with alternatives of equal value should circumstances require.
